Civil Registry
All Registration of Civil
events of a person shall be made at the office
of the Municipal Civil Registrar. Those events
that are reported after 30 days from its occurrence
are considered delayed, hence, applicant shall
be required documentary evidences. Delayed Registration
is subject to posting of 10 days.

BIRTH
CERTIFICATION
Birth Certificate
Records of the complete expulsion or extraction of a product of conception from
its mother, irrespective of the duration of pregnancy, which after such separation,
breathes or shows other evidence of life, such as beating of the heart, pulsation,
whether the umbilical cord has been cut-off or the placenta is still attached.
Procedures
for Securing a Birth Certificate
1. a. Must be the concerned person himself
or any person authorized by him/her;
b. His spouse, his parents, his direct descendents or
guardian or institution legally in-charge of
him, if he is a minor;
c. The court, or proper public official whenever absolutely
necessary in administrative, judicial or other official proceedings to determine
the
identity of the child’s
parents or other circumstances
surrounding his
birth; and
d. In case of the person’s death, the nearest
kin
2. Ask the clerk to look for the birth records.
3. As soon as the register of birth has
been found, go to the Municipal Treasurer’s Office
and pay the corresponding fees.
4. Go back to the Municipal Civil Registrar’s
Office and present the receipt to the Civil Registry Personnel
for them to release the
birth certificate
being asked.
5. Read the Birth Certificate before going home.
If not registered, the following requirements are
needed for application of late registration:
1. Negative results from the National Statistics
Office (NSO)
2. Barangay certification as to date and place of birth
and parents of child to be registered late
3. Affidavit of two disinterested persons to support late
registration
4. Baptismal certificate
5. Voter’s registration records
6. Late Registration fee as per Municipal Ordinance |

MARRIAGE
LICENSING
Marriage License
Record of a special contract of permanent union between a man and a woman entered
into in accordance with law for the establishment of conjugal and family life.
Procedures for Securing a
Birth Certificate
1. A man and woman must be both at the
age of 18 years and above.
2. Must present their birth or baptismal
certificates upon their application.
3. Must secure DSWD Pre-Marriage Counseling
certificates if still 18-24 years of age.
4. Family Planning certificates
from the Rural Health Unit must also be presented
with no age limitations.
5. Parental consent must be signed by parents if the applicant
is between the age of 18-21 years old.
6. Parental advice must be signed by the parents if the applicant
is between the age of 22-24 years old.
7. After securing all the required seminar certificates, they
must go to the Treasurer’s Office for fees.
8. Go back to the Civil Registrar’s Office for release
of the
application duly signed by the Municipal Civil Registrar.
9. After ten (10) days posting, marriage license will be released.
10. The Solemnizing Officer must register their certificate
of marriage within 30 days after solemnization. |

DEATH
CERTIFICATION
Death Certificate
Record of a permanent disappearance of all evidence of life at anytime after
live birth has taken place.
Procedures for Securing
a Death Certificate
1. Must be registered within 30 days from time of death
2. Must register death in the municipality
where the death occurs
3. Go to Treasurer’s Office
for payment
4. Go back to MCR Office and present
your receipt for them to register the death
5. From the MCR the person registering
must go to the Municipal Health Office for the certificate
of death
6. If the dead was embalmed, go to
the Embalmer for the certification of the Embalmer
7. After securing the embalmer’s
signature, the death certificate must be returned
to MCR Office for issuance of registry number and
signature of
the MCR
If not registered, the same procedures and requirements
as to live births are needed for late registration of death
•Issuance of Certified Copies of Civil Registry Documents
- Pursuant to Article 7 of PD 603, birth record shall be issued only to the owner
of the document if he/she is 18 yrs. old, his/her parents, duly authorized representative,
courts or agencies for administrative purpose.
Marriage and Death records can be issued to interested parties.
Security Papers (SECPA) of civil registry documents can be obtained from the
Office of the Municipal Civil Registrar of this municipality or any NSO Field
Offices all over the country.
•Petition for Correction Under RA 9048
Anyone who wish to file petition for correction of clerical error
in civil registry documents or file for change of first name in birth certificate
to personally visit the office of the Civil Registrar were the record is
registered.
Pursuant to RA 9048 clerical or typographical errors are constricted to those
mistakes committed in the performance of clerical work in writing, copying, transcribing
or typing an entry in the civil registry that are harmless and innocuous, w/c
are visible to the eyes or obvious to the understanding, and can be corrected
or changed only by reference to other existing records.
Errors that involve the change of nationality, age, status or sex of the petitioner
are not included from the coverage of RA 9048. Hence, any error that would subsequently
change the age, status, sex or nationality of a person shall be filed with the
proper court and not with any Local Civil Registrar's Office. |

RESIDENCE
CERTIFICATION
Community Tax
Every resident who is 18 years old and above who has been regularly employed
on a wage or salary basis for at least 30 working days is required to secure
a Community Tax Certificate.
Procedures for Securing
a Community Tax Certificate
Community Tax Certificate can be secured at the local Barangay
or at the Municipal Treasurer’s Office. |
