CERTIFICATION
Death Certificate Record of a permanent disappearance of all evidence of life at anytime after live birth has taken place. Procedures for Securing a Death Certificate 1. Must be registered within 30 days from time of death 2. Must register death in the municipality where the death occurs 3. Go to Treasurer’s Office for payment 4. Go back to MCR Office and present your receipt for them to register the death 5. From the MCR the person registering must go to the Municipal Health Office for the certificate of death 6. If the dead was embalmed, go to the Embalmer for the certification of the Embalmer 7. After securing the embalmer’s signature, the death certificate must be returned to MCR Office for issuance of registry number and signature of the MCR If not registered, the same procedures and requirements as to live births are needed for late registration of death •Issuance of Certified Copies of Civil Registry Documents - Pursuant to Article 7 of PD 603, birth record shall be issued only to the owner of the document if he/she is 18 yrs. old, his/her parents, duly authorized representative, courts or agencies for administrative purpose. Marriage and Death records can be issued to interested parties. Security Papers (SECPA) of civil registry documents can be obtained from the Office of the Municipal Civil Registrar of this municipality or any NSO Field Offices all over the country. •Petition for Correction Under RA 9048 Anyone who wish to file petition for correction of clerical error in civil registry documents or file for change of first name in birth certificate to personally visit the office of the Civil Registrar were the record is registered. Pursuant to RA 9048 clerical or typographical errors are constricted to those mistakes committed in the performance of clerical work in writing, copying, transcribing or typing an entry in the civil registry that are harmless and innocuous, w/c are visible to the eyes or obvious to the understanding, and can be corrected or changed only by reference to other existing records. Errors that involve the change of nationality, age, status or sex of the petitioner are not included from the coverage of RA 9048. Hence, any error that would subsequently change the age, status, sex or nationality of a person shall be filed with the proper court and not with any Local Civil Registrar’s Office. |
